If your child requires medication throughout the day to be administered by a member of St Peter's Catholic Primary School staff, please complete the form below and hand it to a member of the office, together with the medication which will be stored securely in the medicine cabinet in the First Aid Room. Alternatively, forms will be available in the school office for you to complete.
